Sealant for PVC pipe through concrete

I'm looking for a good way to seal a PVC pipe going through a concrete wall.

It's to be a buried septic tank with 12" walls, and inlet and outlet pipes of 3 or 4" PVC. The tank will be cast in place.

Your thoughts appreciated.
 
The boots look like good choices. The Polylok and Tuf-tite seals are advertised up to 6" walls. Maybe a trick to getting them to work with a 12" wall?
