Renaming file through Insight problems - SE v16

I am renaming file in revision manager and having problems. I set action to remane and perform the action and no result seem to appear. When closing the window a dialog box appears and says no action has occured do you want to try again. If I select yes it says the file has been created and do you want to override the file. If yes is pushed it makes a copy of the file in my cache with a status of unknown. While all of this is occurring the file never appears on the server. if I delete all items out of my cache it will place a copy of the file with the new name on the server and leave the old file on the server also with the old file name.

We've recreated the file on our local desktop and it does rename the file from there.

This problem has occurred since upgrading to v16.
