question on cross connection 1

Hi there:

Back in the day in college professiona just briefly mention about cross connection. He kind of mention is the situation that different pipe crossing on top of each other. My old co-worker during co-op also mention the same thing. Now, I am working in a really world now. I had talked to some co-worker on this matter. They believe is a situation that just pipe connected in a wrong way. Can someone here can enlight me a bit?
