Thanks for your responses. I've got sample plans from Ohio, Florida, Texas and Mississippi. It's interesting to see the variation in the anchorage zone reinforcing between the four different plan sets. For the longer spans (80 feet +) some use pot bearings under the columns while others provided a "pinned" connection by extending reinforcing from the column up into the cap and having the cap sitting on a little shear key with expansion joint material surrounding it. It doesn't allow for the translation like the guided pot bearing but it would allow the cap to rotate freely. We were thinking of using spreadsheets for our designs. The only tricky part may be the phased post-tensioning that we'll likely need.
