PCA Slab-on-grade Design Charts

I am looking for the formulas used to develop the PCA Slab-on-grade Design charts. I want to put these formulas into a spreadsheet so that I won't have to use those awful charts.

I am using the charts found in "Designing Floor Slabs On Grade" 2nd Ed. by Ringo and Anderson.
