Thanks for the reply. I do have the PDF document. What I am looking for is a filled in document. The pDF doc is a bit confusing.

I can psooibly send you a typical one for A106 GrB if you advise E-mail Address but bear in mind that for piping you also need PMA's for the fittings/flanges etc.
