What would a steel weld dye penetrant test report look like? Any samples or examples out there?

Simple answer, contact a local nondestructive testing (NDT) company and have them send you a copy of a typical Liquid Penetant test (PT) report that they would provide to a customer. Most reputable NDT contractors would do this for your business. I gather from the nature of your question, that you do not have this in-house capability.
