NASTRAN composite failure mode for Hashin criterion

A first ply failure analysis of a composite laminate using Hashin type failure criterion is carried out. The failure results in the .f06 file indicate a "failure mode" which are numbered 1 - 4. As I understand it, it is indicating the dominant failure mode - fiber tension/compression or matrix tension/compression. I have two questions on the basis of this analysis and results.

1) What do each 1 2 3 4 mode represent (i have tried to look into the documentation but to no avail)
2) Even when the failure index is less than 1, a failure mode is reported. What does this imply?
