Are previous editions of this specification "grand-fathered" into the new edition?
 
This standard was originally prepared in 2003 and was revised in 2005, 2007, 2010, and 2012 by NACE TG 231.Each edition had something new.
2012 edition updates and supersedes all previous editions of MR0103.

Does this mean that I have to redo my hardness traverses for my PQRs to the newest edition?
 
No, your welding procedure was qualified to a valid standard.

Which version of MR 103 was followed for hardness measurements, yr 2010 or before.

Appendix C outlining hardness measurement was introduced in 2010 version of this standard. If you see the following clause "C3" in 2010 edition, which states:-
-Hardness surveys performed prior to the issuance of this edition of MR0103 that used the hardness survey layouts in NACE MR0175/ISO 15156 are acceptable. Hardness mapping should be done in accordance with Figures C1 and C2, which show hardness test locations and details,for butt welds and fillet welds.

The 2005 year was followed. There is a hardness traverse listed in this edition as a schematic only. It does not look like a current NACE MR0175 traverse.
