Section D.4.2.2 in appendix D states that the design procedure of D.5.2 and D.6.2 is applicable to anchors with diameters not exceeding 2" and tensile embedments not exceeding 25". This is apparently based on current test data. What does everyone use for anchor rods larger than 2" and deeper than 25"? One of the old methods?
 
Appendix D. IBC overrides the limitations in ACI (See Exception in IBC 2006 Section 1912.1)
 
You're problem might be finding test data for your calculations. I know Hilti has bond strength equations for HY 150MAX and RE 500 that you can use to calculate the capacity but this would be using ASD.
