how to specify internally lined tank correctly

Dear all,
I have a internally lined (Epoxy phenolic) carbon steel shop fabricated 10' diameter tank with 0.29 psig design pressure for raw water service.

Question on how to fill out API standard data sheet, L-21 item # 11:

A). shall I check "yes" to seal weld underside of lap-joints ?
B). Shall I check " yes" to Seal weld roof plates to top angle on the inside ?
C). Shall I specify Figure F.2, details a, f, h, i, or k with outward angle or ring to prevent any pocket that internal lining may not be done well if there is a pocket at the junction?
D). Shall I ask full penetration weld at the roof to shell junction and grind inside weld smootly to prevent crevice corrosion prior to applying epoxy lining at the junction ?
