How Does Concrete Sampling for Cubes/Cylinders and Slump Happen in Your Neck of the Woods

Just interested to see what kind of response we can get on the issue of proper concrete sampling.

As ASTM and BS EN (though in a round about way) say to sample, even for spot samples, to let the first part (0.3 to 0.5 m3) go, then commence spot or preferably composite samples, I have found on my Asian jobs, it is very difficult to get the concrete QC group to do this - they usually take the sample as the transit mixer arrives on site before any concrete "goes".

So what is done in your neck of the woods?
 
No takers, eh. Just trying to see how far off spec various areas go in sampling.
