Referring to UBC 1997 Table 21 - H - 1, what is meant by grout spacing? I have an example where there is an 8 in CMU wall reinforced with #5 at 48" on center vertical. 48" is listed as the "grout spacing" so does grout spacing really mean the spacing of the vertical bars or is the use of 48" just a coincidence here?

It's not a coincidence. The grout is placed in the same cells as the vertical bars to bond the bars to the wall, assure the bars stay centered, add shear area and probably a couple of other things I'm forgetting. You can also grout the other cells if desired, if you want to add more mass or shear area.
