Gate Valve Actuator Leak

Hi All,

“Our hydraulic actuator assembled Gate Valve” started leaking during operation. During the preliminary investigation, it was found that the maximum hydraulic oil working pressure for actuator is 3,000 psi whereas applied inlet pressure was 4,000 psi. However, after dismantling and visual inspection, there was neither any debris nor any piston seal damage found.
What might be the probable root causes apart from inlet pressure? Has anyone come across such Actuator leaking issues in your experience?
What are the preventive measures that we can implement to mitigate such issues in future?
What are the OEM documents we can request to analyze the product qualities in depth and to see if any discrepancy?

 
How was the problem fixed, was the piston seal replaced?
