Fly Ash Test Certificate - Example

Was wondering if anyone had a couple of "example" test certificates for ASTM C618 (Tables 1 and 2) that are used in standard practice? If you do and don't mind to attach - would appreciate it. Please black out location, testing lab, etc. - don't want to know. The testing labs over here in Indonesia don't seem to have any standard format in reporting and I am wanting to get them to standardize it.
