Failure mode analysis 2

I am looking to find documentation about failure mode analysis in order to write a FMA procedure. Does someone knows a good book or web site?

You will have to be a lot more specific on what you want. Failure analysis is an entire field, that many professionals spend their entire career! EXACTLY what do you want to know? What types of devices / technology / etc?

My coleage (mechanical engineer) and me (electrical engineer) have to put Failure Mode and Effect Analysis procedure in place for the equipement that we have for our new petrochemichal plant.

As we will be in start up the plant this summer, we need a simplified FMEA procedure to solbve the problems that will occur at begining. We will revise the procedure after to implement a complete FMEA procedure.

As I am new in the FMEA, I am looking for good books to understand the basis of FMEA and be able to implement such analysis in our plant.

Equipement invalved : Pumps, compressor, centrifuges, boillers, motors, control valve, instruments, analysers, heat exchangers, piping, towers, vessels, etc.

MIL-STD-1629A was the last military standard that describes procedures for FMEA.

Tailoring is the watchword. You could spend a lifetime doing a single FMEA, given the money. TTFN
