Persianger....as skeletron noted, you have not given us much information with which to respond. What is the purpose of your evaluation? Is it for structural capacity? Deflection? Vibration? Rotation? Torque? Foundation? Proximity to traffic? Clearances? Painting? Corrosion Protection?.....there are many reasons to evaluate mast arms.
The bottom line is what skeletron noted.... presumably it's an engineering report covering one or more evaluative criteria. FDOT is not particular about your format unless it is for a specific checklist inspection or similar on a form that they require. If a general engineering evaluation report, as long as it is presented in a logical format with all evaluative criteria, observations and assessments presented in order, and signed/sealed by a licensed professional engineer in Florida, they are usually OK with it.
