Electrical system reliability for Process safety in chemical plants 1

There is plenty of literature on Electrical safety, preventive maintenance and testing but I can not find any information of Process failure incidents causing chemical process failure resulting in serious OSHA reportable incident of hazardous chemical gases or liquid release. Can anyone suggest any reference on this subject? Thanks

I've got a copy and while I haven't read the whole thing, I can say that it is quite a good read and covers a fair number of case studies.
 
Thanks Wayne440 I do remember reading the EPA doc you sent. Juleselec thanks to you also. Perhaps I have not made myself clear. Since the Union Carbide - Bhopal, India incident much has been done for process safety incidents. What I am specifically looking for is what electrical system failures are responsible for maximum process safety issues in Petro-chem industry?
