Dent before Cold Bending Pipe

In a pipe line gas under construction (1440 PSI design pressure), a double joint pipe 42” and WT 0.601” was bent in a cold bending process. Indications were not encountered in the curvature area, but a dent of 3/32" depth in an area no larger than 2” square and close to neutral axis was evidenced. This dent was difficult to observer because was covered by the factory coating.
According to ASME B 31.8, 841.2.3, should the pipe be accepted?

So the kink was in the neutral axis (side) of the pipe, not the intrados?
How did they do that?

Is the kink "in" or "out" of a theoretical round?

I want to be clear saying that the dent no was originaed by the bending process, and it was evidenced in the side of the bent (close to the neutral axis).

According to ASME B 31.8, 841.2.3, just at state (a) (1), must immediately the pipe be rejected? , if not, as it was evalueated according to ASME B 31.8, 841.2.4, (c), should the pipe be accepted?
 
If the dent does not effect flow and no cracks are present from surface NDT leave it alone.
