In the US, it's based on traffic volume. Take a look at Chapter 6 of the NYSDOT Bridge Manual, which provides guidance on selecting barriers and railings. States DOT's generally have pre-designed barriers. AASHTO LRFD Chapter 13 provides design specifications. If you need a design example look at the Minnesota DOT Bridge Manual.
