conditional reset of dynamic system

I have a project that I am moving from Matlab to Simulink but I am experiencing a few difficulties (I am new to Simulink).

I have a systems that need to be reset to another state (just setting new initial conditions) conditional on some value in the system. Is this possible and if so how?
