Concreteing Around Existing Util

I am designing a grade beam will be cast around an existing sub-slab utiltiy.

Anyone have experience doing this? Any recommendations? I'm fine with detailing the reinforcement around the existing utility. I'm more interested in things like:

-Do you oversize the hole?
-Install something like geoinclusion?
-Recommendations on forming the opening?
-Anything else you can add.

Do I need to be concenered about the heat that's generated by the concrete? The grade is not that big... it doesn't qualify as mass concrete.

I take it the grade beam is perpendicular to the pipe. It depends on the expected movements of the grade beam, but usually wrapping the pipe with one or more layers of compressible material will suffice.
