Concrete Spalling after welding at steel connection plate 1

We have a new project where a concrete wall shows spalling right after the end of a steel beam was welded to a steel plate that was attached to the vertical face of the wall. Have someone experience such a problem? or was something to do with the Contractor's installation procedure that cause this spalling?
 
We don't have a specific procedure to follow other than to make note on the plans that the contractor needs to allow "x" amount of time for curing and to use weld procedures to avoid excessive heat build-up in the concrete.
