To clarify my question: I would like to find out what is the average yearly percentage of concrete pavement rejection at the time of construction due to poor workmanship in the United States?

Have you tried contacted the American Concrete Association? You can also contact the Engineering Library of a University by you. Referenece librarians are wonderful resources.
