Does anyone know possible causes of this damage to concrete sidewalks?

Google 'concrete popouts' and you will find that there are several causes of popouts, all related to the aggregates used.

Those look more like bugholes, (the technical term), don't they?

If so, not quite convinced that's damage, per se.
