SW: 2014.

I don’t recall what to tweak in the “calloutformat.txt” file to show THRU instead of THRU ALL.

Also, is there any way to show Decimals in place of fractions for Hole Callouts? (Military Specs call for it).

The difference is not in the calloutformat file. The difference is in the modeling. Get in the habit of selecting "up to next" rather than "thru all" for your extruded cut. Then the callout on your drawing will say "thru", not "thru all".
 
To show decimals instead of fractions, Go To
System Options
Hole Wizard/Toolbox
Configure
Hole Wizard
Choose which standard(s)

Revise fractional to decimal.
You may have to play around with this to get it in the form you want.
This may have to be a system wide change for it to show up on everyone's drawings, I'm not sure.
