Axisymmetric Elements with Nonaxisymmetric Loads

How can I appling non-axisymmetric loads or displacement to an axisymmetric model? I use bilinear asymmetric-axisymmetric elements (CAXA4N), but I also want to impose the asymmetry to the loads. (I'm using ABAQUS)
 
Check the Verification Guide in Abaqus documentation, it features an example problem named "Cylinder subjected to asymmetric pressure loads: CAXA elements". You can download input files for this example to see how it should be defined.
