ASME Cat. D Weld including Repad

Working out NDE for a new project.
Client requires all Cat. D welded to be fully UT inspected. There are several nozzle which require reinforcement pads. The question is.. Is the inner repad reinforcement weld considered part of the Cat. D weld, or is just the nozzle to shell / head pressure weld the Cat. D weld.

What's the item to be welded, i.e. pipe or pressurize equipment, and what's the applicable code applied?
 
Does Cat D talk of butt welds only? If yes, then no need to consider fillet welds.

Anyway, if UT is done for almost all joints, then why leave the RF pad to shell weld out of the scope of UT?
