RE: Appendix D, D.5.3.4:

With a headed anchor rod, should Abrg be the gross area of the head (or nut), or should it be the net area of the head (Gross head area minus bolt area). I've been assuming it is the net area....

Yes, it is the area of just the head bearing on the concrete

Yes...Abrg is just that area that's bearing on concrete.
