Concrete pump boom in the photos, so I am guessing the formwork was insufficient to support a deck placement.

Another construction collapse

Various descriptions of the collapse seem flaky. It looks as though there was an opening in the level beneath the pour requiring a variation of form work to cover the span and that portion is what collapsed.
