The requirement D.5.2.3 of ACI318 state:

"Where anchors are located less than 1.5hef from three or more edges, the value of hef used in Eq. (D-4) through (D-11) shall be the greater of Ca,max/1.5 and one-third of the maximum spacing between the anchors within the group."

When it says "where anchors are located..." is it refered to the group of anchors or for each single anchor of the group?

For the group, but it is obviously for individual anchors within the group.
 
It is for anchor group in pedestal alike conditions, the distance limit is applied to indivisual anchor as correctly pointed out by StrEIT
 
It is for both. If you are checking one anchor, it is of course for one anchor. If you are checking a group of anchors, then the outer anchors of the group may be too close to the edges and thus affect the Anc.

Agree, your statement is also true, oe sounds better. The distance is measured to the center line of any bolt, or bolts, that is closest to the free edge.
