anchor bolt

I have interior column footing and trying to design anchor bolt for that. Top of footing is 1'-0" below slab on grade - finish floor elevation 0'-0" and footing is 5 ft wide. i am trying to check anchor bolt for tension and shear as per ACI Appendix D. There is no pier. Now for concrete breakout strength i just have to consider footing dimension to calculate area ?? as there is no pier do i need any lateral ties around anchor bolt..?

looking at concrete breakout, for the calculation "ANo" (I think, no code book here) on a very large area compared to the anchor group I use the area as (spacing of anchors + 1.5 hef + 1.5 hef) for both sides.
so if you had a four bolt group:
anchor group is say 8"x8" and you "hef" = 12", then

ANo = (8" + 1.5*12 + 1.5*12) x (8" + 1.5*12 + 1.5*12) = 1936 IN^2
