AISC Volume II Connections - Example 17 1

I was looking through some of the connection design examples in the AISC Volume II Connections Green Book and how they calculated the gusset stress fb at the beam interface. In Example 17 on page 7-132, they use fb = 3*M/L^2. However, in Example 21 on Page 7-148, the calculate fb = 6*M/t*L^2.

I am confused as to why they are different and not sure if there is an error in Example 17. Many of the examples I have looked at outside of this book use an equation similar to Example 21. If anyone could help, it would be much appreciated.
 
6*M/(t*L^2) is correct. There's an errata pamphlet with my book that shows this (resulting in a required weld size of 1/4, not 3/16). The errata is dated 1/1/94.
