4-20 mA Current Loop Isolation

We are adding a 4-20 mA output and was wondering if there is a common practice to ground isolation. Is it standard for the transmitter to isolate ground or the receiver or is it common practice to not isolate the grounds at all?
 
Isolation improves common mode noise rejection. Stay isolated if possible.
