1) Please don't SHOUT - it's considered rude here and won't get you many answers
2) This is not a free engineering site
30 A requisition is simply a document listing what you want and including all the specifications, drawings, SDRL, scope of supply, delivery specifications, commercial conditions etc etc. your purchasing department should be able to supply you with a typical.
