Pendulum amusement ride snaps at major connection, causing carriage to fall to the ground. Several fatalities.

I tried to see the video but got "geo location blocking" so I fired up my VPN service and hit it from Sweden. Same problem.
So I switched to a VPN server in my service located in India!
At which point Chrome ratted me out.

The main pylon clearly fails at point of fixity with the central hub. Fatigue and corrosion.
I've never liked the look of those pendulum rides.

There are similar ones without a counterweight, which presumably isn't nearly so taxing on the arm that holds the riders.

Looks to be a fairly brutal change of section from the axle to the main shaft. Stress concentration point for sure.
Most of the one's I've seen don't have counterweights, but presumably this reduces the structural loads on the main A frame supports.

That was such a... well behaved failure. It's fortunate it didn't fall off landing on an edge or it would've created a bunch more carnage.

It looks like the shaft was bolted onto the main cetral point, but seems to have broken above the flanged joint if you look at the two photos below, the break point seems to be well above the colourful collar.
the "reinforcement" of the flanged joint looks rather odd to me - I wonder if there was some previous damage or the designer recognized that the joint was under a lot of moment.
