failure of beam

after analysis of a reinforced concrete 10 storey building with seismic analysis in ETABS ( base shear method)
some beams fail as it is red coloured
but i cannot find the reason for failure
the shear strength(tauV<taucmax) is ok ...also percentage of reinforcement < max allowed
Could anyone tell me what else can be the reason for failure

Sometimes red color indicate that you may very close to failure but not failed. But any way pls attach your summary envelop sheet of a said beam produced by etabs
 
usually if stress level is over 95% of allowable limit , it will indicate in red colour.
