Small holes in elevated slabs

Does anyone know what is typically used to fill small holes in elevated slabs, say a 3" diameter max? Could it be as simple as roughing up the edges and filling in with a non shrink grout such as Duragrout?
 
I would also use a surface bonding agent.
 
Your "simple" approach is all that is required for small holes. The grout will be its own bonding agent.
