Is there any publication, such as an ACI document, that makes a connection between the height of rack storage and the need for a superflat floor? What determines when there should be a superflat floor?
 
Not my area of expertise, but your standard warehouse racks can accomodate quite a bit of imperfection in the slab flatness. Are you worried about rack instability because one post is a 1/2" or so higher than the next? I don't see this as a problem, but others may have had specific problems...

Usually flatness of a floor is more dependent on things like special machinery or equipment that must be installed on a floor, or for other unique situations.
 
Look at the type of material handling system. If it is an automated system, this could drive you to superflat. Another consideration is the height of the racks, but again from a material handling approach.
 
American Concrete Institute (# 117) provides standard tolerances for concrete construction and materials.

45/35 CONSIDERED 'FLAT' TYPICAL WAREHOUSE WITH AIR PALLET USE, ICE RINKS, ETC.
65/40 CONSIDERED 'VERY FLAT'
100/50 CONSIDERED 'SUPER FLAT'

ALTERNATIVELY,

THE MAX VERTICAL DEVIATION OVER A [24 | 600] WIDTH SHALL NOT EXCEED
2.5mm (quality FM1)
3.5mm (quality FM2)
5.0mm (quality FM3)

THE MAXIMUM VERTICAL DEVIATION OVER A [10FT | 3.0M] WIDTH SHALL NOT EXCEED
3.0mm (quality FM1)
6.0mm (quality FM2)
8.0mm (quality FM3)

FLATNESS TESTING SHALL BE UNDERTAKEN WITHIN 24 HOURS OF SLAB FINISHING


generally automated systems require super flat floors.

Not worried about rack instability. Worried about whether the forklift can insert the pallet into the higher levels of shelving if the floor does not meet superflat tolerances. I was under the impresssion that there is a certain shelf height above which need superflat floor with forklift, but I am not sure.
