Our company recently completed a fire sprinkler project in a existing nursing home. Some of the spray-on fire proofing was disturbed by attaching beam clamps to the structural steel.
The facility is fine with this, but the Engineer is looking for us to "touch-up" every where this situation occurred.
Has anyone ever encountered this before?

Yes-NYC was quite strict on this.

would be similar to penetrating a rated wall, needs to be properly protected.

I commonly require these corrections on spray applied fire resistive materials, especially in high-rise buildings.
