For nozzles inserted thru the vessel wall, see Figs UG-40(d), (e), Figs UW-16(g). Preferred practice ordinarily is to follow Fig UW-40(e-2), so that the thickened portion of the nozzle extends at least equal to the limits of reinforcement normal to the vessel wall. This portion is made thick enough to provide the needed reinforcement, per UG-37 rules.
Select the bore such that the thinner portion of the neck meets UG-45 and UG-27 / UG-28 rules. Ideally, the bore will be selected to match the attached piping, but it is often not practical to determine this. Selecting X-Stg bores will typically work.
The forgoing generally applies to nozzles attached by butt-welding, Fig. UG-40(f) and similar. In practice these types are usually per Fig-UW-40(e), but with added lip for the buttweld. These types tend to be expensive, but are sometimes required.

The problem with sloppy work is that the supply FAR EXCEEDS the demand
