S&C Fused Cutouts

Hello, I've been having a problem with S&C 14.4 Kv/100 A fused cutouts not dropping free when the fuse link breaks.
All it takes is a SLIGHT touch with a hotstick at the bottom of fuseholder and the tensioner pulls the link free and they drop out.
When inspecting them after removal all pivot points are free of any debris and move freely.
They are used on 4160 Y with 25 A links installed.
Anyone run into this before.
