Is it a good industry practice to remove and test PRV everytime it lifts in service but satisfactorily reseats following that without leakage? How can one confirm that set pressure has not changed during the PRV lifting? If there are reference reg this in any of the standards, pls update my knowledge.

Test after successful lift and reseat is not required. I would only note the lift in that PRV file, and have the PRV shop do a teardown [opposed to 'Test ONly' when it becomes time for "Test & Reset" of the PRV. Teardown is to clean inside of PRV.
