Protection of Underslab Medical Gas pipe

I am trying to find out how to protect medical gas piping which will be installed underslab.

NFPA requires that it be placed in a protective enclosure. One of the plumbing codes requires a split enclosure.

How is this done?

Who makes/manufactures the split enclosures?
