Today the US Chemical Safety and Hazard Investigation Board released its final report on the September 2003 biotech facility explosion at the Ohio Isotech facility.

It is worthwhile reading on facility planning, pressure vessel failure, HAZOP ananlysis and the inherent dangers of this exotic process.
