Losing Icons

Happy Friday ya'all!!

Heres the 411:

After installing Service pack 4.0, ( not me, but my fellow worker) and afterwords, all her icons were missing from Microsoft explorer and getting a fastener error " unrecognizable file format."

Can anyone advise, please?

How was the SP downloaded/applied? It could be that the it didn't download properly/fully and corrupted the install, but that's just a guess.
