Anybody used Insulated Concrete Form (ICF) for 4-5 story condos and wants to share design/construction experiences would be greatly appreciated.

You design it the same way as if you were using regular form. If the coarse is 16 3/4" high, that means the lease amount of horizontal steel you can have is @ 16 3/4" O.C. Each coarse has up to 10 spots to put horizontal rebar.
 
I've used it for 3 and 4 storey apartment buildings. Treat it as a 6" typ wall. OK for exterior walls, but a little pricey for interior walls... There are a couple of prescribed booklets for construction and you should check with the supplier regarding any particular requirements they may have. There are likely several dozen suppliers and they may have their own prescribed reinforcing. I've used it with wood and hollowcore floor systems...
