I need information about mono or tri phase transformers tests form which test can be done to how to do them ...

You don't say where you or the transfomer installation is located, or what insulation medium is used, but the IEEE C57.12.* series of standards may be applicable.

The international standard for this topic is IEC 60076 which has lots of different parts regarding the different topics of transformer application, design and test.

For transformers other than oil immersed some details may be described in other IEC standards.
