How to morph/blend/sweep one cut shape into another,

Can anybody help,

I'm looking to create a tape former. I need to take a flat tape and form a tube inside a solid block of material

the tape will be placed between points 1 + 2 of the input shape and then slowly curved inside the block to form a tube as shown in the output shape
with the ends of the tape at positions 1+2 on the output shape and then finally closed using a die of the final tube diameter.

I'm at a loss to see exactly how to perform this blend/morph/sweep. My google-fu has let me down and I cannot find any tutorials that have done anything similar

If it is possible to show me how to create this I would be extremely grateful
