Hole in Pipeline -Dl DN 600

Can anybody tell me how this hole formed in this pipe line ?

Bit difficult to tell but looks like a manufacturing defect to me ( slag or similar inclusion)

Or a corrosion hole.

Is it possible the pipe was dented from outside first, and then corroded?

The pit initiated from the surface that is shown in the photograph. Refracory material is visible. Was it a case of improper refractory application?

Material, service, years, required for further guidance.

thank you every one for your replies ,
its a ductile iron pipe with cement lining .DN 600
pressure in pipe around 8-12 bar
flow 500-600 m3/h

the pipe was laid 3 meters below the ground
service ; more than 10 years

recently there was a leak .while trenching we found heavy rocks

Length of pipe?

You are doing the right thing, putting a patch is fine.

Sometimes you weld a box with a valve at 6 o'clock which drains out the water during welding and you close it after you finish welding.
